Background: The head of household is a mom with two children. The children are girls, ages 9 and 6 years old. Katherine (name changed due to confidentiality) is a 30 and is assiduously working towards completing a Bachelor’s of Science from Chamberlin College of Nursing and is expected to finish within a year. Katherine became homeless due to her adoptive mother saying that the home was getting overcrowded and that she could no longer afford to house her and her two children. Katherine is currently working on the prerequisites and then will complete licensing to become a registered nurse. In the interim, she has been working to secure part-time employment. Objectives: Katherine will achieve goals in income enhancement, stable housing and self-sufficiency.

Background: The client is a single, 41-year-old female with two female children ages 18 and 12 years old. Both children are enrolled in school. Monika (name changed due to confidentiality) and her children became homeless as a result of fleeing from a violent relationship. She had received assistance from Journey’s PADS until referred to Catholic Charities in July 2019. enrolled in Harper College and completed a certificate in phlebotomy December 2019. Monika has secured a full time position and is preparing to exit the housing program on or before the lease ends in June 2021. Objectives:Monika will achieve goals in income enhancement, stable housing and self-sufficiency.
Family #1 and #2 Goals:
●Will maintain housing and abide by housing agreements. ●Will utilize budget and resources by having a monthly savings and find opportunities for additional income. ●Will be in contact with Case Manager once a month. ●Will find affordable housing in the desired area.
