To continue to serve as a Eucharistic Minister for the next three years please read the following and fill out the form below.

Since ministry is a call both from God and the community in which it is exercised, it is appropriate that the choice of ministry and renewal of the term of service be mutually agreed upon by the individual and parish.

Please prayerfully reflect up your commitment to Eucharistic Ministers as it relates qualifications and responsibilities. If you discern to recommit to the Eucharistic Ministry, please complete this form by October 2, 2017.

Eucharistic Minsters will serve for a period of three (3) years, after which time an evaluation should occur - November 2017 - Dec 2020.

Please note that it is helpful that our parish keep accurate records of each Eucharistic Minister’s training, ongoing formation, and their term(s) of service.
